Summit-Horizon Dental Laboratories Warranty Policy

Summit-Horizon Dental Lab will, at its election, repair or replace any dental restoration, fabricated by Summit-Horizon Dental Lab, which proves to be defective in workmanship or materials as follows, provided that the restoration is seated within 60 days of the invoice date:

• Porcelain to metal (PFM), full gold/alloy, all ceramic, single unit composite, and single unit inlay and onlay restorations are warrantied for 5 years from the invoice date.

• Zirconia products have a 5-year warranty from the invoice date unless specifically stated otherwise.

• Removable prosthetics including; oral appliances, removable partial dentures, dentures, and immediate partials are warrantied for 90 days from the invoice date. NOTE: If the try in steps are suggested and skipped per the office's instruction, no warranty will be provided.

• Fracture on metal bridges and zirconia bridges are warrantied for 5 years from the invoice date.

• e-max® bridges have no warranty against fracture.

• Maryland bridges have no warranty at all.

Summit-Horizon Dental Lab warrants that a remake or credit will not be applied under the following conditions:

• If a remake or credit is requested because the customer requests a tooth shade different from the

Original shade request.

• If a remake or credit is requested due to treatment plan change or materials change from original request.

• If a remake or credit is requested for immediate/surgical partials or dentures or treatments with healing extractions.

• If a remake or credit is requested because the appliance fits the model but does not fit in the mouth.

• If a remake or credit is requested and the remake reason is not specified, and original materials or appliance(s) are not returned.

• If the manufacturer's minimum thickness requirement is not met and the lab is instructed to fabricate the prosthetic regardless, the product in question will not be warrantied.

• Summit-Horizon Dental Lab's remake or credit policy will not apply to any account past due. Summit-Horizon Dental Lab reserves the right in its sole discretion to refuse to accept any new cases and process remakes or credit cases until the balance is paid in full and the account is current.

Summit-Horizon Dental Lab can amend the remake or credit policy at its sole discretion and without prior notice.

Repairs both fixed and removable performed on work manufactured originally in another dental laboratory are specifically excluded from the Limited Warranty. Summit-Horizon Dental Lab does not warrant work initially fabricated by another company.
